School Registration

Parents must register their children enrolled in grades 6 - 12 for the 2005 - 2006 school year. Parents may register their children on Monday, June 20th, at the Terrell Middle High School Cafeteria from 4:00 until 7:00 p.m. Parents unable to attend registration on this date must come by the school to conference with the principal between the hours of 9:00 a.m. and 3:00 p.m. no later than June 27, 2005.
This registration is mandatory and parents who fail to register their children by June 27th will be reported to the proper authorities or agencies. Students will not be allowed to enter school until registration is complete.

The following information will be required to complete registration:
1. Social Security Number
2. Name of medical insurance and policy number
3. Name of Physician and telephone number
4. Medical documentation of any problem that interferes with the student's ability to participate in physical activities or remain in a classroom for a sustained period of time.  (Example: urinary problem, required medication at a specific time of day, etc.)
5. Emergency contact phone numbers
6. The name of persons you authorize to sign your child out of school
Note: Parents or the person authorized by the parent(s) must report to the school to sign a student out of school.
7. Directions to your home from the school.
8. The bus number if your child rides a school bus.
